Your new role

As a locum Adults Social Worker in this hospital team, you will play a crucial role in delivering high-quality social work services to adults with diverse needs. You will collaborate with healthcare professionals to ensure holistic care and support for their service users.

• Conduct comprehensive assessments and develop person-centred care plans.
• Facilitate safe and timely hospital discharges.
• Provide advocacy and support to service users and their families.
• Work collaboratively with multidisciplinary teams to ensure the best outcomes.

What you'll need to succeed

• A recognised qualification in Social Work.
• Registration with Social Work England.
• Proven experience in adult social care within a hospital setting.

• Accountable for delivering a statutory social work service

• Have compassion and empathy to support individuals who need our services

• Work in partnership with colleagues to deliver a high standard of social work practice

• Work hard to support people to regain their skills and independence following their hospital admission wherever possible.

• Have access to a strong and dynamic social work service with clear leadership
