Key Responsibilities:
- Provide day-to-day support to clients, including assistance with daily living activities and household tasks.
- Develop and implement individualised support plans in collaboration with clients and other professionals.
- Monitor and report on clients' progress, ensuring their needs are met, and any issues are addressed promptly.
- Encourage and facilitate clients' participation in community activities and social events.
- Maintain accurate and up-to-date records of support provided.
- Work collaboratively with other team members and external agencies to ensure comprehensive support for clients.
- Adhere to all company policies and procedures, including safeguarding and health and safety guidelines.
Requirements:
- Previous experience in a support worker or similar role is preferred.
- Excellent communication and interpersonal skills.
- The ability to work independently and as part of a team.
- Compassionate, patient, and empathetic.
- Strong organisational and time-management skills.
- Relevant qualifications in health and social care are desirable.
- A valid driving licence and access to a vehicle is advantageous.
